INJURY OF THE WEEK
Posterior ankle impingement 
The posterior ankle joint is made up of the talus and the tibia.  When the toes are pointed down (plantarflexion) the talus bone slides forwards relative to the tibia to create a smooth movement.  If this glide doesn't happen, the joint space will be compromised when the foot is moved into dorsiflexion. 
When the joint we between the two is compromised, it can cause a connection between the two bones resulting in micro trauma.  Repetitive micro trauma between the talus and tibia may result in a bony spur (os-trigonum) forming on either the superior-posterior aspect of the talus or the inferior aspect on the tibia, resulting in pain. 
Closing down of the joint space between the talus and tibia may cause entrapment of soft tissue in the posterior ankle.  This is often caused following an inversion injury where the foot turns inwards resulting in compression of the tibiotalar ligament (posteromedial soft tissue impingement). Posterolateral soft tissue impingement is also possible with compression of posterior ankle ligaments, joint capsule or anomalous muscle.

The hip hinge is a basic movement pattern that everyone must have! Doesn't matter if you're young, old, athlete, or a student, you must learn this as it is the foundation of movement.  A patient few weeks ago came in describing "firework" like sounds from her left knee, which after exam there was a lot of instability in the knee.  Through rehab/strength & conditioning of the hip and re-learning to properly hip hinge, squat, and lunge she is functioning even better than before as she can perform single-leg squats and single-leg deadlifts.

So what is the Hip Hinge?
The hip hinge is how you should be picking objects off the ground, it’s how you should go from sit to stand, it’s how you should move furniture, it’s how you should do most activities throughout the day.  Without the hip hinge, you're either loading your knees or your lower back and preparing for injury.
It is a posterior (backwards) weight shift through the hip joint, where the hips become the axis of movement between the upper and lower extremity through a neutral spine.

Benefits of Hip Hinge:
1. Utilizes the entire posterior chain
2. Spares the knees and spine
3. Allows for transfer of energy from lower to upper body

* Moderate Trunk Lean
* Pelvis Moves Posteriorly
* Hip Flexion

Why your shoulders and thoracic spine are rounded? 
Many people suffer nowadays from this alignment which is caused mainly by 2 🗝muscles: 
1⃣the pectoralis minor and 2⃣the lower trapezius muscle (ascending part of the trapezius). 1⃣The pectoralis minor runs from the coracoid process down to ribs 3-5. It will become adaptively short when sitting at the desk, working with computers, favor exercises such as the bench press or other pressing exercises etc., which most people do! Being in poor positions and using the pecs will cause this adaptative shortening and tightness. When the pec minor is positioned short it will pull down on the scapula tilting it forward. It also will inhibit (weaken) the rhomboids, lower trapezius and serratus anterior which will lead to muscular imbalances!
2⃣The lower trap runs from vertebra T7-T12 up to the spine of the scapula. Normally this muscle pulls down on the scapula from the back creating a balancing act with the pectoralis minor for the scapula position. But if the pec minor is getting tight and short throughout poor positioning the lower trap gives up and gets weaker and weaker!
.
This poor alignment we are talking about can even lead to mechanical problems in the cervical spine or shoulder joint as it makes it very difficult to use the glenohumeral joint in centrated neutral position. That's why it is essential to work on it!
. 
Nevertheless there are some other important muscles which can lead to protracted shoulders... I will talk about in the following posts of this series!

💕4 years ago, at this time of year, I was only 20 years old. I was pursuing pre-med and I was thinking of going into medical school but it didn’t seem right for some reason. I wasn’t really sure what or where I wanted to go in life or how to share my gifts and talents✨

I decided to see a school counselor finally,but before that, I remember very vividly taking ten minutes before I left the house that day to meditate and pray. I specifically asked source to bring guidance into my life and to lead me into the direction that I needed to go. I went to the counselor, and she recommended a school that she just visited a few days prior! It was for a chiropractic college just an hour away! I literally had an epiphany and things clicked!  I’ll never forget this day. On that same day, I was seeing my chiropractor and I happen to meet someone who was attending that school when I went to my appointment! It was the day that everything just clicked and I knew in my heart that this was my purpose! I called the school that same day and I started the process.

The funny thing is, that I thought of being a chiropractor before...it crossed my mind. But immediately for some reason, my mind would come up with some crazy excuse that I could never be that because I wasn’t strong enough or whatever the funny reasons were. So I would quickly ignore that thought and continue in my confusion for where I needed to go lol. It’s funny looking back, the things we tell ourselves. I don’t know why it finally clicked on that day, maybe because I was open to it, but I am SO GRATEFUL for where I am now. Being at the end of the chiropractic program now, it all seems so surreal. Exactly two months until graduation!

Now, I love going everyday to work with people and I love seeing the results that they get! It’s the greatest gift!

The hip hinge is a basic movement pattern that everyone must have! Doesn't matter if you're young, old, athlete, or a student, you must learn this as it is the foundation of movement.  A patient few weeks ago came in describing "firework" like sounds from her left knee, which after exam there was a lot of instability in the knee.  Through rehab/strength & conditioning of the hip and re-learning to properly hip hinge, squat, and lunge she is functioning even better than before as she can perform single-leg squats and single-leg deadlifts.

So what is the Hip Hinge?
The hip hinge is how you should be picking objects off the ground, it’s how you should go from sit to stand, it’s how you should move furniture, it’s how you should do most activities throughout the day.  Without the hip hinge, you're either loading your knees or your lower back and preparing for injury.
It is a posterior (backwards) weight shift through the hip joint, where the hips become the axis of movement between the upper and lower extremity through a neutral spine.

Most of the postural stress is managed by decreasing the stress the spine takes throughout the day. No amount of adjusting or massage will fix your back if you accumulate a lot of poor posture throughout the day.
.
While not all poor postures are avoidable, you'll really decrease your risk of developing pain from poor posture by adding these movement snacks into your routine.
.
🔑 Fixed neck rotation using a swivel chair. If the neck is already tight, using this method is great at releasing tension in the lower neck and upper back. To loosen up the lower back, fix the torso in place and rotate just the lower body.
.
🔑 For flexion/extension, try to segmentally flex and extend to hit each area of the spine. Try to focus a little more on the CT and TL junction.
.
🔑 For lateral flexion, reach up towards the ceiling and take a deep breath. This will open up the lateral chain and decrease tension in the flank.
.
🔑 Seated hamstring stretches. Ensure you have an anterior pelvic tilt as you're trying to stretch the hamstrings here.
.
🔑 Hip Flexor stretch is a key stretch to for anyone sitting. With this hip flexor stretch, I try to reach the leg back as far as comfortably possible while not over extending at the low back.

Perform 15 to 30 seconds of each drill on each side.

A 29-year-old professional came to see our kinesiologist Gracielle 5 days before going on an overseas holiday with a severe back pain that was aggravated whenever he moved. He reported being unable to travel to work or even sit up to use a computer without experiencing pain. To manage the pain, he was taking 8 to 10 pain killers a day. During the initial consultation, he expressed his lack of hope about recovering on time for his trip that was planned all around snowboarding activities and was contemplating the possibility of cancelling it altogether. He was asked to provide a pain severity from 0 (no pain) to 10 (extremely painful) and at that point he rated the pain as 8 when doing anything mobile.
During the session, it was uncovered that his back pain was directly connected to mental and biochemical imbalances. The imbalances were corrected and a couple of supplements had to be introduced to his diet to reduce his stress levels related to his work life and to improve muscle endurance. After the session, he could twist his torso without pain and only felt slight pain when bending forward to touch the floor. The pain reduced from 8 to 2 on that session. He started to take the supplements and the following week he was on the slopes without any pain at all.

My warmup this morning, along with some convos I've been having with friends about power lifting, inspired this DM. Always train in the sagittal plane, that's all you're gonna be able to access and perform in. Leave it..and well..hello, injury. Only train the big 3 lifts and you're training your way to a short career. Train weird, leave the box, (respect the process of course), explore your end-ranges, expand your resiliency.

On the flip side, it's also pretty unconventional (in a good way) to listen to your body, go slowly, move with intent, and train unilaterally 🤷🏽‍♂️. It doesn't need to be fast and outrageous (it honestly probably shouldn't be 🙈) for it to be unconventional. Once again simply reiterating the point that you get out what you put in. Doing the basics extraordinarily well is one of the most unconventional training methods out there. Choose wisely.

These muscles (transverse abdominis, obliques, pelvic floor, diaphragm) are main stabilizer of the body, thats why it is very important to work on a proper activation of these important structures!

Before working on activation of the core muscles I always release the rectus abdominis muscle with several myofascial treatment techniques as you can see in the picture.

A tight or shorten rectus abdominis can lead to muscle imbalances and therefore to lower back problems!

There was a huge outpouring of folks that suffer from muscle tension or "knots" through the midback and wouldn't ya know, rounded shoulders and tension through the midback are very much inner-related. .

Biomechanically🤓- "rounded shoulders" describes an internally rotated and protracted shoulder. Generally, the anterior musculature has become tight or overactive in relationship to the posterior musculature (heeey 👋🏻rhomboids, mid-lower trap and external rotators)

Folks with "rounded" shoulders may experience:

🔻looking like they have forward rounded shoulders🤔
🔻limitations with overhead movement🙆🏻
🔻pain/discomfort through the midback🤕
🔻numbness/tingling down the arms, into the hands⚡️
