Radiological imaging studies show alterations in the physical structure of cervical muscles in patients with chronic mechanical neck pain. ATROPHY, PSEUDO HYPERTROPHY, AND FATTY REPLACEMENT of the cervical extensor muscles (Andary et al. 1998, Elliot J. Et al. 2006, Kristjansson et al. 2004)
.
Most notable are in the suboccipital and deep multifidus muscles due to their high density of type 1 fibers, but also superficial muscles such as semispinalis capitis muscle.(Boyd-Clark et al. 2002, Cote P et al. 2004).
.
Neck pain is associated with disturbed neural control of the cervical muscles. Changes in muscle behavior include heightened activity of the superficial muscles such as sternicleidomastoid and anterior scalene muscles during cervical flexion and upper limb movements. Superficial cervical flexors also appear to be slower to relax following movement or muscle contraction as does the upper trapezius muscle in response to repetitive upper limb movements ( Falla et al. 2004, Fredin Y et al. 1997, Nederhand MJ et al. 2002).

THIS MEANS that deep cervical muscles get inhibited with neck pain in static and dynamic upper body movement because superficial muscles remain overactive to compensate!

Many people could benefit from some simple exercises to improve activation of the diaphragm. When performing at a high intensity, the ability to supply oxygen via respiration will affect your performance no matter the amount of strength work you have previously done 🤔

The diaphragm has a huge role in stabilizing the core: it works with the internal and external obliques, quadratus lumborum (QL), pelvic floor, and transverse abdominus(TVA)

This is briefly showing our process 👆🏾for diaphragmatic breathing and TVA activation. This is after soft tissue work for the backline now creating tone/sensitivity in the frontline. We ARE NOT cueing rib pulling or lumbar flattening..we are just working from a rested position staying passive with no specific pushing or pulling from the client

My fingers are under the lower back to feel the erectors to feel how much stimulation is going on. This is not our first session together in this post, but sensitivity in this area is still a huge priority

If you have AIR(inhale) in place, the body then has pressure to protect you with…the exhale will help to reset the position by pulling the ribs and pelvis towards each other fighting extension

This part of the process can all be done passively… before learning to handle ACTIVELY with more of a mechanical approach of drawing in and actively pulling in the ribs

We like to show that you can protect yourself just with the breath. Learn to LIVE in the frontline then work on harder postures and loaded positions

It’s cool to see the body adapt to the new pressure/position in just a session… but 1 hour won’t defeat 23 hours… so the mindfulness it takes to break old habits is just as important

Tips:
▫️cough or laugh with your hand on your lower stomach… thats your diaphragm!
▫️6 X 6: Aim for 6 seconds inhale, 6 seconds exhale
▫️Inhale: through the nose getting your lower stomach as big as you can
▫️exhale: out the mouth (“puckered lips”, athletic forceful exhale) until you have no more air
▫️put 1 hand on your chest, 1 hand on your stomach(or a weight)…only move the hand on your stomach

The Rotator Cuff Muscles #1 - The infraspinatus

We're going to go into the muscles of the shoulder which are specifically known as the "rotator cuff". The shoulder joint is a ball and socket joint but is a pretty unstable one as the "socket" isn't that deep to contain the "ball". The shoulder joint relies heavily on the muscles surrounding it to maintain stability - these are known as the rotator cuff muscles.

The first one is the infraspinatus, which pretty much sits on the bottom part of the shoulder blade.

Function: It laterally rotates (externally rotates) the arm and stabilise the shoulder.

Stretch: Try the shoulder stretch that most of us have done during our school days. Take your right arm across your body, use your left arm to grab your right elbow and pull it towards the left. 
Strengthen: You will need a prop for this one so try using a bottle of water. Lie on your side, knees can be bent at 90 degrees in front of you. With your top arm, hold the water bottle, bend it at the elbow and place the upper arm against your side - your forearm should be parallel to the floor. Slowly bring your forearm up so that your fist is essentially pointing directly up to the ceiling whilst keeping your elbow tucked into your side - the elbow shouldn't really be coming off the side. Bring the forearm back to its starting position.

Faced with this case we have to consider:
1) if there is an infection👻; 2) if the material is sufficient for consolidation to occur (no signs of release) 3) will it be necessary to change the stability strategy?

In this case,  we opted for the reverse sural flap rotation ... a versatile flap and very indicated in the skin cover defects in 1/3 distall of the tibia and, mainly, in the foot.

No caso em questão, optamos pela rotação de retalho sural reverso... um retalho versátil e muito indicado nas falhas de cobertura cutânea em 1/3 distall da tibia e, principalmente, no pé.
