Case of the day:
This very nice gentleman came in to our office with a chief complaint of a broken tooth. Exam and x-ray revealed a broken cusp and tooth decay on tooth #18 . Tooth #19 was a tooth that had a root canal completed when the patient was in pain at a prior visit.
A discussion was had with the patient. Tooth #19 was in need of a full coverage crown to prevent tooth fracture (crowns are needed on root canal teeth in the back of the mouth to prevent this) and #18 was in need of a restoration as well. We hated to cut tooth #18 down for a crown because most of the tooth was in good shape (pictured below), but we knew a composite filling would not hold long term because of where the break was (on a cusp, the chewing surface)... We decided to place a crown and a CEREC onlay! Picture is below. Rather then prepare the entire tooth, a precision milled CAD/CAM Onlay and crown were milled with our CEREC machine in the office in a single visit! No impressions were needed and the final porcelain restorations pictured below were delivered in the same day! The patient was happy to have the hole fixed, as well as a nice crown to protect his root canal tooth on his first molar!

This very nice gentleman came in to our office first back in January with a chief complaint of a throbbing tooth ache in his front tooth. He had bitten into some hard food and heard a loud pop. As you can see the patient has a serious history of bruxism (teeth grinding) as seen by the crack in his tooth. Sure enough our pulp tests showed he had developed symptomatic apical periodontitis (a toothache in the ligament around the tooth resultant of the crack)
We discussed a treatment plan, save the tooth with a root canal, remove it and place an implant, remove it and place a partial, or remove it and do a three unit bridge. The patient wanted to save his natural tooth and elected to go with a root canal even though he knew the prognosis was guarded long term because of the crack and the tooth grinding.... A root canal was completed and the patients symptoms resolved completely! No more ache 😁
Low and behold a little less then a year later the patient was eating again, and heard a very loud pop that he described as a gunshot going off in his mouth 😵 the tooth had broken off at the gumline!
After some more discussion, and deciding that cutting down two healthy teeth for a 3 unit bridge was not really in his best interest, he decided to have a dental implant placed!
Tooth #8 was extraced atraumatically, the root removed, the bone on the facial aspect of his tooth was preserved completely, and a Straumann 4.1x12 Bone level tapered implant was placed using a 3D printed pilot drill guide made by our Cone Beam x-ray and CEREC machine for precision placement. We elected to cover the implant after placing bone graft material rather then load it immediately because of the patients grinding. He will have a brand new tooth in just a few short months and is currently wearing a temporary!

The before and after of restorative work Dr. Thompson completed on a patient. This patient presented with multiple areas of decay and failing amalgam fillings with recurrent decay evident on X-rays. (She was also having discomfort with the tooth on the bottom right in the first picture) We were able to get her in quickly and using our cerec technology, we were able to complete the crowns in one day. White composite fillings were also placed in several areas. Went to New York only to come home again! Sunday Brunch at @maysvillenyc was absolutely perfect!!! The restaurant's namesake is derived from my hometown and the Kentucky port city from which whiskey was first shipped out of the state 🥃🐎 And they got it SO right. From the food to the horse paintings AND the asparagus foil wallpaper in the bathroom! Whaaa! 😍Thank you to the wonderful staff for such an amazing homecoming/going! ❤️
