Went for my second opinion gynecologist. He is strongly against a hysterectomy and he will look over my op notes from my last lapro but doesn’t think I need another. After a bunch of tests and examinations he diagnosed me with IC, myofacial pain syndrome, and pelvic muscle dysfunction. If you pee a little when you cough, laugh, or sneeze, you likely have weak pelvic floor muscles.

⚜Geçmişte kadınların yüzlerce pelvik taban egzersizi yapması tavsiye edilirdi. Şimdi bunun yanlış olduğunu biliyoruz ve en faydalı sonuçları alabilmek için kas güçlendirme ve dayanıklılığını arttırma konusundaki mevcut bilimsel veriler temel alınmalıdır.
⚜Pelvik taban egzersizleri genellikle her gün yapılması gereken güçlendirme egzersizi olarak tavsiye edilir.  Egzersizleri günde 2-3 set yapmanız yeterli olacaktır.Ancak 1 sette yapacağınız tekrar sayısı sizin kapasitenize göre değişiklik gösterir.
⚜Aynı şekilde önerilen egzersiz sayısını aşarsanız kaslarınızda aşırı yorgunluğa neden olabilirsiniz ayrıca idrar ve dışkınızı yaparken gerginlik hissedebilirsiniz. Ne kadar çok egzersiz yaparsam o kadar çabuk iyileşirim diye düşünmeyin! Bu nedenle fizyoterapistinizin ön gördüğü tekrar sayısına bağlı kalın.

⚜Kegel egzersizlerine başlamadan önce mesanenizin tamamen boş olduğundan emin olun. Bu çok önemlidir. Mesaneniz yarım ya da tam dolu iken Kegel egzersizleri yaptığınızda büyük olasılıkla ağrı hissedebilirsiniz ya da idrar kaçırabilirsiniz.
⚜Pelvik taban kaslarınızı sıkılaştırmaya konsantre olun. Kegel egzersizlerinden en verimli sonucu alabilmek için karın, kalça, bacak gibi diğer kaslarınızı sıkmaktan kaçınmalısınız.
⚜Konsantrasyonunuzu arttırmak ve hareketlerden en iyi sonuçları alabilmek için egzersiz süresince doğru şekilde nefes alıp vermelisiniz. Yani asla nefesinizi tutmamalısınız. Bu sizi rahatlatacak ve hareketleri tamamlayabilmenizi sağlayacaktır.
⚜Egzersizleri yaparken kalça ve karın kaslarınızın gevşek olduğundan emin olun. Kalçanızı sıkmayın, nefesinizi tutmayın ve bacaklarınızı bir araya getirip sıkmayın.

Human pelvic biomechanics is such an interesting area of study as it is incredibly unique in comparison to other animals. As far as we are aware of we are the only animal on the planet that have movement at the sacroiliac joint (SIJ). Besides humans, all other SIJ’s in the animal kingdom are fused joints - there is no movement taking place between the ilia and sacrum. A lot of evolutionists believe that our mobile SIJ is what gives humans our signature bipedal gait. This movement is very small (somewhere between 1 and 1.6 millimetres). Despite this movement being ridiculously small, it’s incredibly important movement as it helps redirect loads and forces between the upper body and lower body. Most injuries in the body are due to the body not absorbing and distributing forces correctly. Over time this can overload certain muscles, ligaments, joints ect… Which can lead to ‘wear and tear’. Reinforcing balanced movement at the SIJ is essential for spine health. 
The small movements of the sacrum (nutation and counter nutation) have a direct influence on the mechanics of the spine. It’s generally good to check an individual’s pelvis and sacrum if they are dealing with lower back discomfort. From a functional perspective, the lower back and pelvis are directly connected and heavily influence each other. 
One of the main stabilisers for the SIJ is a pelvic floor muscle called the levator ani (I’m sure you can guess what the other function of the levator ani is). The pelvic floor is an interesting area, especially in the world of pilates as most of the focus is on building strength at the pelvic floor. ‘A weak pelvic floor needs to be strengthened through strength work’ … I don’t agree.

Improving how an area functions normally involves the ‘releasing of some muscles’ and exercising others. Strength work is really important but the irony is if a muscle is testing ‘weak’ (what ever that means) it might really just be ‘tight’ or in ‘spasm’. If you give the muscle the capacity to contract more efficiently again (improving the neuromuscular connection) when you re-test the muscle, it can test as ‘strong’ (what ever that means).
