There are 4 ways we categorize a cesarean birth: - scheduled (example: an appointment surgery for Wednesday morning, 10am, for breech presentation) .
- unscheduled / nonemergent (example: you're supposed to have that cesarean Wednesday at 10am but your water breaks on Tuesday. You're not contracting or dilating, and you just ate lunch, so we wait 8 hours for an "empty" stomach. Another example might be an induction of labor intending a vaginal delivery, but a cesarean is recommended hours later if the induction was not successful) .
- urgent (example: a cesarean was recommended after a prolonged labor because mom or baby have developed fevers/ infections and are remote from delivery. We might bump some scheduled cases to get you into OR. Not an emergency, but it's best to deliver sooner rather than later). This category means from "decision to incision" we have a 30-60 minute window.
- STAT / emergency: True obstetric emergencies when mom or baby life is in danger. Rare. (Example: umbilical cord prolapse, serious fetal heart rate decelerations, placental abruption). This category means we have 0-30 minutes from "decision to incision." Instead of waiting for a spinal anesthetic to kick in, general (you're put to sleep) anesthesia will usually be used. .
I've learned over the years of doing patient intake and 😳when a patient tells me her last birth was an emergency CS that unscheduled / urgent / STAT cesareans often all feel like emergencies to the patient. Because it's unexpected, and not the plan, and scary. It's an emergency to them, even if it's not an emergency in my little computer boxes.

Every year we celebrate a natural birth, a story that takes place in the most primitive surroundings. Mary, giving birth to the Son of God in a stable, that infamous image portrayed recurrently in our culture, familiar to even the non-religious. Yet how is it that beyond Julius Garibaldi’s 1891 painting of Mary and Joseph slumped in raw exhaustion, we have never seen a ‘real’ depiction of birth biology, particularly of Mary in upright, ecstatic primal instinct that such an environment would have helped facilitate?
Risking controversy to use universal characters to portray the ultimate ‘birth undisturbed’ amongst other mammals in a dim and lowly environment, suggests to modern woman that often in birth, less is more. An ideal lies really in the balance of nature and medicine, but to depict Mary in the powerful moment of bringing Christ Earthside, into the calm and steady hands of an actively participating Joseph, protests any notion that the humbling, creative power of woman is anything but as awe-inspiring as the creative hand of God himself.
