Lip augmentation can and should be customized to the desires of the patient while still respecting that patient’s unique anatomy. Multiple fillers can be used depending on the properties desired (softness vs firmness, shorter vs longer-acting, etc). Often different fillers can be “stacked” over multiple treatments to get the desired look. The above patient wanted more of a “pout” and wanted to plump the upper more than the lower lip. Her goals were achieved using 1cc of Juvederm Ultra. This filler is composed of a naturally occurring composing (hyaluronic acid) and should be expected to last 3-6 months.

Occasionally I hear people dismiss aesthetic breast surgery as “simple” or “bread and butter” plastic surgery. I would offer it should demand the same level of precision planning and execution as all aesthetic procedures. Poorly done primary augmentation can lead to expensive secondary surgeries. Revision surgery presents some of the most challenging cases in the field. It requires a provider experienced preferably in both aesthetic AND reconstructive surgery. This out of town patient presented to me after multiple surgeries with another surgeon. She had several issues including capsular contracture, implant bottoming out, and a “snoopy deformity” where a breast sags lower than the implant. In addition to correcting these issues, the patient was interested in an increase in size. Using multiple operative techniques I was able to improve her cleavage, her shape, as well as the feel of her breasts. When a patient greets you at her 2 week followup with happy tears and a hug, well, it just doesn’t get any better than that for me. Couldn’t wait to show her early results.

By this time, you’ve probably heard all about how fat can be used as a filler in various parts of the body whether it be to create new curves (eg. buttock augmentation) or to restore lost volume (eg. facial rejuvenation). I see many patients - such as my patient above - who have stretched out an ear piercing hole. This can make it uncomfortable or even impossible to wear earrings. With a simple office procedure under local anesthetic, the hole can be repaired and the ear eventually repierced. (1) A common presentation of a stretched ear piercing hole. (2) The first step in the repair is to carefully and strategically remove tissue to create a fresh wound. (3) Under magnification, the hole is meticulously closed in layers both from the front and the back of the ear. (4) The ear can be repierced as early as 6-8 weeks and once healed, the scar is nearly invisible. (Photo taken at 3 months)
